Incomplete forms will not be considered, so please fill out all of the information that I’ve requested of you! I'll ask for reference pictures, if applicable during our initial email communication after receiving this form. Please don't send them unprompted as I may lose track of them in the email shuffle. Lastly, as I am not only a tattooer but am currently undertaking additional Entreprenurial endeavors and finding contrast between work and rest please allow 3-7 days for my reply to this form.

Please let me know if mornings or afternoons work better for you. Please take notes that afternoon sessions beginning at 1pm are only 2.5-3 hours long, I don't begin day sessions that late in the day. All day sessions begin at 9am.

Please be advised that in most cases, consultations are not necessary to book you as all of the information that I need will be in this inquiry form. However, if we've never worked together or if you feel like your idea will be better expressed in person, I am beyond happy to book a consultation to meet with you!
